Reporting Forms

Reporting forms can be printed out and sent by post or as a pdf file by email. To document the reported case, the pdf data can be saved on the reporting party's server. For a follow-up report (forms 2a, 2b, 2c), the assigned PEI number should be entered in order to enable rapid allocation.

Online Notification

Due to technical problems, online reporting of transfusion reactions and incidents will not be possible in the near future. The Paul-Ehrlich-Institut is working on a solution. Please use the following reporting forms temporarily.
